Here are some details below about the insurance plans accepted at AAPHC, as well as assistance provided for those who are uninsured or underinsured at this time. Self-pay patients are also welcome!
All Major Medical & Dental Insurance Plans Accepted at AAPHC
AAPHC accepts most major insurance plans, including Georgia Medicaid Managed Care Organization such as Amerigroup, WellCare, and Peach State.
For dental insurance, AAPHC accepts most major dental plans including DentaQuest. If you ever have any questions about the insurance plans at AAPHC accepts, please reach out to your Provider's office.
Please note that all AAPHC patients are responsible for payment of any co-payments and/or co-insurance at the time of service.
Sliding Fee Program Available for Qualifying Patients
As a Community Health Center, AAPHC is dedicated to serving our Southwest Georgia residents and no one shall be denied medical care due to inability to pay. We never turn our patients away!
Through our Sliding Fee Program, we offer assistance to low-income and uninsured patients. AAPHC's Sliding Fee Program allows us to reduce or "slide" the fees for the care of you or your family. It matches the cost of your care with your ability to pay, based on income and household size. Sliding Fee Program patients may also receive discounts on prescription drugs through our pharmacy benefit programs.
In order to apply, a patient must provide one month's worth of proof of income for all household members. The amount of the patient's co-pay will be based on the household income. Please feel free to contact one of our locations for more information regarding this wonderful program. Just ask for our Sliding Fee Program information and application!
Please note that patients using this program will be expected to pay their Sliding Fee Program co-pay at the time of the visit.
Self Pay Patients
Individuals who do not have insurance, and do not qualify for the Sliding Fee Scale program, can still receive care at AAPHC. As a Self Pay Patient, however, you are responsible for full charges. Please feel free to contact one of our locations and learn more about the Self-Pay option for patients.
