For over 40 years, AAPHC has built an organizational culture on 3 Cs – Compassionate Care, Community Connections and Continuous Improvement. These three principles guide everything we do as we strive to fulfill our mission of providing ‘quality health care to everyone’.

We recognize that health care is more than treating illness or seeing patients once during their annual wellness visit. It is about building relationships, strengthening communities, and helping our patients and our communities live healthier lives. At its core, we know community connections play a vital role in improving health outcomes and ensuring individuals and families have the support they need to thrive.

Health Happens Beyond the Clinic Walls

While regular checkups, screenings, and medical treatments are important, many factors that affect health can occur outside of an AAPHC facility. Access to nutritious food, safe housing, education, transportation, employment, and social support all influence a person's overall well-being and their ability to access quality health care.

One of the ways we extend care beyond our clinic walls is our Mobile Access Care Units. Our units are out in communities providing primary care, screenings and immunizations. Bringing quality health care directly to patients and communities is one of the many ways we stay connected to the people we serve throughout southwest Georgia.

By working alongside local organizations, schools, churches, businesses, and community leaders, we can help address barriers that may impact a person's ability to receive care.

AAPHC was founded when community members, business leaders, and local stakeholders came together to address a critical health care need in southwest Georgia. From the beginning, we recognized that strong community partnerships are essential to building healthier communities. Today, we are honored to partner with health care organizations, businesses, nonprofits, schools, faith-based organizations, and community groups to help meet the needs of the communities we serve. From health fairs and educational programs to community events and outreach initiatives, we are committed to providing health education, resources, and access to care.

Strong community connections also help build trust between providers and patients. Our providers actively participate in community events, educational programs, health fairs, and outreach activities. Through these connections, AAPHC becomes more than a place to receive medical care. We become a trusted health care partner invested in the long-term health and well-being of our patients and communities.

Trust encourages patients to seek preventive care, keep appointments, ask questions, and take an active role in managing their health. It also helps create open communication, allowing providers to better understand patient needs and deliver more personalized care.
