Through education, support, and engaging activities, families gain the knowledge, tools, and encouragement needed to make lasting lifestyle changes and build a healthier future together.

Why join?

Who’s eligible to join?
- Parents must be:
- 18 years or older
- Self-identify as Black
- Live in the same household as child
- Have prediabetes or at risk for diabetes
- Child must be:
- 8-15 years old
Program details:
- 16 weekly family sessions
- 4 assessments that include vital signs & surveys
