We believe healthy habits start at home. This family-focused program brings parents, guardians, and children together to learn simple, practical ways to improve their health and lower their risk of Type 2 diabetes. Through education, support, and engaging activities, families gain the knowledge, tools, and encouragement needed to make lasting lifestyle changes and build a healthier future together.

 Why join?

  • Education about health habits to prevent Type 2 Diabetes
  • Monetary and non-monetary incentives for one parent and one child during intervention

 Who’s eligible to join?

  • Parents must be:
    • 18 years or older
    • Self-identify as Black
    • Live in the same household as child
    • Have prediabetes or at risk for diabetes
  • Child must be:
  • 8-15 years old

 Program details:

  • 16 weekly family sessions
  • 4 assessments that include vital signs & surveys
